Our Process
Wanting to be a new client?
Inquiring:
If you are interested in becoming a new client, please submit the New Client Inquiry form below. Our intake specialist, Jackie, will review your inquiry and be in touch. She can help to answer any questions, and connect you with a provider who best fits your needs, availability, and insurance.
New Client Paperwork:
After you have been connected with a provider, Jackie will send you our new client paperwork via our therapy notes client portal.
Couples & Family: Clients interested in couples/marriage or family therapy services will be set up using only one person’s information. Only one person will complete paperwork, and be billed for services.
Children: We are legally required by the state of Ohio to ask for and obtain the following guardianship documents: adoption, custody, legal guardianship, power of attorney, caretaker authorization affidavit, and foster care.
Scheduling:
Once all of the paperwork is submitted and processed, Jackie or your provider will reach out to get you scheduled.
Billing & Insurance Benefits:
Before your first session, we will verify your insurance, and Jackie will provide you with an estimate of your out-of-pocket costs based on your benefits for mental health in an outpatient setting. All benefit information we provide is an estimate only. It is your responsibility to know your insurance carrier, eligibility and benefit information prior to receiving services. We are not responsible if your insurance does not cover, or if your insurance is through a payer that we are out of network with.
After your session, we will submit claims to your insurance, and charge your card on file for any client responsibility due. If claims come back from insurance showing different benefits, or if we have any issues with your insurance coverage, you will be notified as soon as possible.
Insurances Accepted: The insurance plans accepted vary by provider.
Commercial: Aetna, Anthem (BCBS) BlueCross BlueShield (Includes out-of-state plans), Benefit Plan Administrators (BPA), Cigna, Frontpath, Medical Mutual, Paramount, United Healthcare (UHC), Optum
Medicaid: AmeriHealth, Anthem, OHIO Rise, Buckeye, Caresource, Humana, Molina, United Healthcare (UHC)
Other: Buckeye MyCare, Caresource Marketplace, Molina Marketplace
No Insurance: If you do not have insurance we can offer self pay options. Our self pay pricing ranges from $80 - $150 per 60 minute session, and varies by service and provider. When we connect you with your provider(s) and determine the service(s) we can give an exact price.
